
One of the things we enjoy most about being Care Coordinators is discovering resources that can truly make a difference for the families we serve. Recently, we had the wonderful opportunity to attend an informative presentation by Nathan Lampenfeld, Special Assistant to the Secretary of Aging for the Pennsylvania Department of Aging, introducing the PA CareKit. We were so impressed by this free resource that we wanted to share it with you.
The PA CareKit was created to support anyone caring for a spouse, parent, family member, friend, or loved one. Whether you're just beginning your caregiving journey or have been doing it for years, it offers practical tools, educational information, planning worksheets, and guidance to help navigate the many challenges caregivers face.
One feature we especially liked is the Understanding Your Needs Quiz. By answering a few questions about your caregiving situation, you'll receive personalized recommendations and resources tailored to your family's needs. As circumstances change, you can revisit the quiz and receive updated guidance.
The CareKit also includes helpful resources such as medication trackers, care coordination worksheets, emergency planning tools, home safety checklists, financial planning information, and guidance on topics including Medicare, Medicaid, home care, dementia, long-term care, caregiver stress, and end-of-life planning.
While resources like the PA CareKit are an excellent place to start, we know that finding information is often only the first step. Understanding which resources apply to your unique situation and knowing what to do next can sometimes feel overwhelming.
